Before Your Visit

• Arrive 15 minutes early to complete new patient paperwork. Our team can assist with forms or retrieving health records.

• Bring your health insurance card (if you have one). If you’re uninsured, we can assist with applying for coverage.
• Bring a list of medications you are currently taking (or bring the medications themselves).
• Prepare any health-related questions for your provider.
• Provide your pharmacy’s name, address, and phone number for prescriptions.

During Your Visit, We Can

• Introduce you to your primary care provider (PCP).
• Explain how to schedule appointments and access services.
• Arrange lab tests, follow-ups, and referrals.
• Discuss your treatment plan, including medications and next steps.
• Review prescribed medications and proper usage.
• Confirm dates and times for upcoming tests or appointments.
